Form Wizard Pro - Choose From List, Option Group, Options, US States, Territories, Canadian Provinces

I'm creating a form using Form Wizard Pro, and I need a list that gives me all of the US States, US Territories, and Canadian Provinces all in one list, and grouped. Does the "Choose from List" feature support the use of optgroup in the option list? Also is there already a default list control that would give me US States, US Territories, and Canadian Provinces all in one list?

Otherwise what are my options? Can the choose from list control be extended? I did see where I can create a custom submission handler, but that not sure if that is what I need or not.

Hi,

It doesn't currently support option groups, but I'll log a request in my project tracker and consider that for possible future enhancements.

i7MEDIA has a download with some free Form Wizard Questions, it may have a state list, not sure if it has the territories or canadian stuff but you could add those in. Even if you type them in manually, you can then export the question and re-use it so you won't have to type it again.

I don't have any immediate suggestion for the grouping. There is not currently a way to make it do that. You could preface the Canadian ones with a prefix like Canada -

One other question, I just watched the video demo of using FWP to develop a survey and in the video, you could associate the instructions with a particular question but for some reason my instructions show up on all the pages instead of just above the question they relate to. Is there a way to fix this?

Hi,

Use an Instruction Block question and you can position it just like regular/real questions. It sounds like you are using the main instructions rather than Instruction Block questions. You add an Instruction Block the same way you add a question.
