Delay time for a Contact Form to show on a Page

Delay time for a Contact Form to show on a Page

I need help with delaying a contact form showing up after a specific amount of time. I have a video that I want new users to watch. Once it ends or near the end I would like a contact form to appear so they can email me proof of watching the video. I need some sort of timer to allow this section to delay until a specific time.
