Is it possible to style the Survey to achieve the layout like in this page?

I like the grouping of the questions on the same page. Do not want to create multiple pages.

I guess the answer is no.

Not sure how to do this in the code without some hooks from the portal.

I agree that this type of styling ability would be really cool, at least for the paid Form Wizard Pro. Joe, if you ever feel like taking a crack at this, it would really be nice to be able to group portions of the forms using <fieldset> and <legend> like they did on that survey (with each fieldset and legend having its own custom css class prompt). Here's a nice article about doing this, and the associated styling. The CSS workarounds are no doubt out of date though.
