It is a difficult task to try to parse the html and look for the first image and it is difficult to make an html fragment of a given length without missing any closing tags or making malformed html, therefore for auto-generated excerpts we just convert to plain text.

So the best thing to do is to edit the excerpt manually and put whatever you want in there including image instead of leaving it blank and letting it be auto-generated.
