create custom form

Hello, I am searching for the way to create support form in my web page using mojoPortal with one text box and several memos and with submit button which would send form by e-mail. Something like this  http://www.encrypt-easy.com/uninstall.asp

Is it possible somehow to create such form using mojoPortal and send it my e-mail?
