I am getting this error:
2011-09-29 19:42:42,014 ERROR xxx.xxx.xxx.xxx - en-US - /FormWizard/FileUpload.aspx?mid=45&pageid=43&rid=cba7d649-7817-4f3d-94c5-f4df273094b9 - mojoPortal.Net.Email - could not find file for email attachment
The attachments are being successfully uploaded to /data/sites/1/Attachments
Also, when it emails the form, it appends the following message right after the form contents instead of inserting a line break after the form contents. This makes it very hard to understand. See example below:
Name: John Doe A form has been submitted, click the link below to view the submission: http://mydomain.com/FormWizard/ViewSubmission.aspx?mid=45&pageid=43&rid=cba7d649-7817-4f3d-94c5-f4df273094b9
Where it starts with "A form has been submitted,..." should start on a new line.

I will look into this today and post again as soon as I've had a chance to investigate it. If needed I'll put out a patch for Form Wizard right away.

This is now fixed. If you download Form Wizard Pro again, the only file that changed was sts.FormWizard.Web.UI.dll in the /bin folder
It should fix the attachment problem and it also adds a line break as requested.
